INFORMATION AS FAILURE TO COMPLY WITH SOME ITS DIRECTIONS MAY RESULT IN YOUR EXAMINATION BEING DELAYED
Please can you bring the following with you to your
appointment:
Two
forms of identification (one of which must bear a photograph). You will require
your photograph identification to enter Regus House and will not be seen without it.
A
list of any medication you may have been prescribed or have taken for your injuries, including any bought at the pharmacy
The
pre-examination questionnaire if one was sent to you by your solicitor/insurance company. If not then please click
on the icon for a list of issues you may wish to consider prior to attending your appointment.
You will be advised in your confirmation letter,
how long your appointment will be.
You can bring a friend or relation for support
or to assist with translation. They will also require photo identification to
enter the building. If you could notify Legal Medicine Consultancy in advance
that you will be accompanied and by whom, this will ease the process.
There is no provision for prolonged waiting
so please aim to arrive approximately 5 minutes before your appointment time.
There are no child care facilities so please do
not bring children with you unless they require to be examined.
If you are held up or unable to attend for
any reason please contact the Legal Medicine Consultancy as soon as possible so that alternative arrangements can be made
if necessary.
Legal Medicine Consultancy or Regus House will not
be responsible for paying travelling expenses. You should speak with your solicitor
or insurance company to ensure reimbursement of any costs before you incur them.
